HEALTH INSURANCE:
The Staffing Health Insurance Program is administered by JLBG Health -
Affinity Health Plans. With more than 30 years of experience administering
health plans for employers, associations and membership organizations, you can
be confident in their ability to provide you with affordable and attainable
health insurance coverage.

This coverage is optional. If you elect coverage you will pay your premiums
directly to the company. There is no payroll deduction available for this option.
OTHER SUPPLEMENTAL COVERAGES:
Supplemental insurance coverages such as level term life, critical illness,
short term disability and cancer insurance are offered through Colonial Life
and Accident Insurance Company. For more information contact our office. These
coverages are also optional but would be deducted from your paycheck.
401K:
This voluntary plan is designed to supplement your retirement savings. In
order to participate in this program, you must work for Free State for a minimum
of one (1) year and at least 1,000 hours. Your contributions to this plan will
get automatically deducted from your paycheck. If you wish to be a part of
this program, please contact us to determine your eligibility.
PAID DAYS OFF:
For those unpaid holidays, sick days or just a day off we offer Paid Days
Off. For every 400 hours you work you will accrue one 8 hour day. Restrictions
apply so please ask for more details.
In addition to these benefits, Free State also offers a First Job Bonus
